PARENTAL AGREEMENT:  

I register my child, _______________________________, for STAGE RIGHT THEATER PRODUCTIONS, SPRING Workshop, 2012.  I understand that STAGE RIGHT THEATER PRODUCTIONS is not responsible for any injury that may result from my child attending STAGE RIGHT classes or rehearsal.  I also give STAGE RIGHT Productions full permission to videotape or photograph my child for production and publicity purposes.  All videotapes and photographs are the sole rights of STAGE RIGHT and may be used for publicity purposes. 

I understand a pro-rated refund is available until the end of the 3rd class.  After that time, there are NO REFUNDS.
