PARENTAL AGREEMENT
I register my child, _________________________________, for STAGE RIGHT THEATER PRODUCTIONS Fall 2010 Musical Theater Workshop.  I understand that STAGE RIGHT THEATER PRODUCTIONS is not responsible for any injury that may result from my child attending STAGE RIGHT classes or rehearsal.  I will inform the Stage Right staff in writing of any pertinent information (medical, etc.) they should know about my child.

I also give STAGE RIGHT Productions full permission to videotape or photograph my child for production and publicity purposes.  All videotapes and photographs are the sole rights of STAGE RIGHT and may be used for publicity purposes.  I understand that refunds will be available on a pro-rated basis until the end of the 3rd class.  After the third class, there are NO REFUNDS.

NEW!  PLEASE NOTE THAT STUDENTS are PERMITTED ONLY TWO ABSENCES during the semester.   Any students with excessive absences from classes, as a result of absences, arriving late or leaving early - will need to meet with his/her director to discuss participation in the program’s performance.
